I doubt it will be a cause for rejection, people move often. Though if you want to be on the safe side you can ask your sponsor to edit the address, its relatively easy for them to make edits or at least add a comment.

I'm inclined to believe that your ILR has been approved as the language is "I am pleased to confirm".
I doubt the case worker would be pleased about telling you that your application has been rejected!

As long as it has the banks stamp, it’s ok. I have used that type before for my extension. Just ensure it has the stamp. Also you can get your statement from any branch of your bank.
