I had tracking number on my NCS receipt. There is paragraph where NCS worker will write your tracking number. Don't worry.

Yes. It will be written on your confirmation letter which you will get from NCS worker.

Is your husband Slovak? If yes then there is option to apply for his lost Slovak citizenship and then you can apply on base of his EEA citizenship. The OPs husband is now British. Even if the husband can get his other EU citizenship back, he is will not be considered an EU citizen in the UK because...

Are you from A8 country? If yes, then you have PR automatically after 5 years but if you want apply for Citizenship then you have to apply for PR card anyway. Just send them all paperwork since you are here and then you can apply for citizenship as soon as you receive that card.

Nope. You should be able to apply as soon as you'l get your PR but I would do it through NCS because they will call HO to amend your status on PR card.

NCS told me that in this case they will contact HO and ask about your time in UK and if they can write actual date when you received your PR. Just go for it and book NCS or speak with they via phone.
