If you have a valid passport the stamps in the passport will prove your stay. You don't need anything else. You need all valid and expired passport to be sent. If your latest passport is expired, then you will need extra documents like P-60, bank statements, council tax bill etc to prove your stay....

You should use the new forms if you apply on or after 1 October 2013. However, we will continue to accept applications made on previous versions of these forms up to and including 22nd October 2013.
