You can pay on one form, and include the main applicant's information. Ensure all the application forms are submitted together with that fee form.

No, a letter from the university is not sufficient. If UK NARIC's Statement of Comparability does not state the course was taught in English and the PBS doesn't award points for English Skills for this degree then UKBA will not recognize the degree as being taught in English.

The simple caution shouldn't affect your naturalisation application as it's over 3 years since it was given. However, you should declare it on the form.
