Confused why you need to explain the illness differently unless it affects your pay or absence limit out of UK.

Just take an absence letter for all 150 days and that should be it.

They cannot accept the application and process your personal details without your consent (GDPR) to verify the information. Also a good reason to reject the application and get more fees. You take printouts and get the HR team at previous employer to stamp it if they do not issue letters in any form other than email. If thety will not stamp it then you might have some issues proving validity.. Worst case I would just submit the digital letter online and take printout to appointment. ...

If you have spent more than 30 days in a year UKVI will not (highly likely)apply discretion of their ability to ignore the need for absence letter from previous employer. You HAVE TO report change in passport number.
They MIGHT let you in with old passport kept with new one but they do not have to.

Get your passport in your maiden name and change your name by deed pool then apply for a new british passport in the new name.
