You will need to find evidence. The reason this is asked for, is to determine and verify that you have NOT been absent for 2 or more years and therefore possibly lost your ILR status. Any other residence documents such as council tax, are only for those under the EU &EU Family route as their passports don't get stamped.

Apply using married name as in the new passport. Marriage cert indicates previous name. There wont be any issue.

No they cannot. Only UKVI can revoke ILR and it would have to be something pretty big for them to do that, ie deception, serious criminal offences etc.

2. She doesnt need a letter. She only fills the absence information in the form. Thats it.
