Presumably you applied for your first spouse visa AFTER you husband got ILR. You can only therefore apply in Set(M) as you do not hold a PBS dependent visa to be able to apply on Set(O).

As a PBS dependent, she can only apply on Set(O). She is the main applicant then. Her 5 years residence starts from the date she entered the UK. For all applications. Appying through NCS does not 'speed up' the process.

Highly unlikely that you will get a passport before mid November. Just the MN1 process takes a couple of months and you need that approval before a passport will be issued.
