I wrote a consent letter. But for

A parental responsibility order, care order, adoption order or special guardianship order. I think this is for those parents who are separated and divide kids care and custody.

For Number (3), shall I put YES But she are living at the address since May 2018. So it is less than 5 years. Is it ok to write the same address in the previous address section as we cannot live this section blank.

Thanks. Why you want to apply on the 26th June. You can apply now on the basis of 28 days. Yes they may charge you extra of uploading the documents. If already uploaded then dont think so.

What documents have you accumulated including for supporting documents for staying here.
