If you have all your tax status in good order, then you can include the SA302 and P60s as additional documents. I don't believe they are mandatory.

If you had gone through this during your initial Tier2 application then it doesnt matter. Even otherwise, it shouldn't matter as you are picking the same university only

If all the 3 choices are providing 10 points then it doesnt matter. You have gone through this when you got your Tier1 for the first time. So, don't worry about it.

Just call up HMRC and request your employment history if you have been working as a full time employee or ask for SA302 if you have filed Self Assessment in the past.

You would get it in 5 to 10 working days by post. I got my SA302 and PAYE within 7 working days
