just make a fresh application mate and if possible write a cover letter. i dont think it will be necessary to mention about previous letter.

but if you want you can give ref number of the old letter and explain them that you are putting all the documents which they mentioned were missing.

if they ask you if you were cautioned etc then you have to come clean. better be honest then to get rejected and have a red stamp in your passport or their immigration record.

deerox as far as i am aware you both should be given same lenght visas as you both are partners and submitted the aplications together. you need to complain and resend your passports to them with a cover letter and they will update your visas hopefully.
