The form you need to fill in is Application form VAF4A - Settlement and the fee is £585. You can find other useful information here and on UK Visas.

Transactions of the amount you mentioned should be explained.

On the other hand transfers that are made from a credit card, I don't think it will get you anywhere even if you explained it. This is because funds available from credit cards would not count towards your total income in the first place.

3 years ... yes ... but I think it is the minimum length of time each referee should have known the applicant for.
