Not sure what do you mean by "By Salary Only". But as long as your bank statement corroborates net salary of your payslips you are fine. Make sure to get all your statements stamped(if possible). To avoid any confusion at CW side, you can also mention salary credited date in your employer letter too...

Does letter head contain other address information of your company or it only contains company Logo?
