The two choices are not mandatory. Answer all questions it’s not tricky. For 5 years you don’t need to provide documents showing your spouse being British. There may be cases where grandparents were British and applicant therefore might be British already, without knowing it. Nothing to worry about.

As I understand it ILE (applied for outside the UK) is equivalent to ILR (normally applied for inside the UK).
As you pointed out, it's not really a visa that expires, which is correct ILR/ILE is "indefinite"
So on the face of it, and as already advised, can apply for naturalisation without issues.

Think of it like a bill you paid late and you had to pay a late fee.

The wording on the website isn't clear enough.
Spouse needs 5 years to gain ILR.
Once on ILR she could apply immediately as spouse of BC for which she has to prove 3 years of residency.
Common misunderstanding.

There’s noting mandatory to do, but recommend you inform your employer, bank and council electoral registry , otherwise only as/if requested. For example in a form where it ask for citizenship state British, you don’t normally have to prove that.

Visa nationals for the Uk still need a visa. ETA is a travel authorisation mainly for non visa nationals. You still need a visa for the UK.

All you need to do is to get the referees to sign the forms. They won’t be contacted by UKVI. The form can be downloaded once you have submitted the application. Anyone receiving income from any source is potentially liable to tax. Google HMRC the tax authority.
You have presettled so no paperwork required, when you apply for for full settlement you need to prove 5 years residency.
