I was in a similar position, if you ring the HMPO office directly rather than the call centre mailine they are generally much more helpful. Just google the particular office's number and speak to them. Even though they say 6 weeks when I rang them , i had BP within 2 working days of calling them, al...

I also used two engineers and I wrote in this format

Engineer (IMeche accredited)
Engineer (IET accredited)

I wrote Professional Colleague for both.

my application was accepted.

You have to only submit the current foreign passport. Any expired passport that she used to enter the UK does not need to be submitted. yes only the the first time you have to enter via the country you applied the visa from. Subsequent trips can be to other countries.
