A signed letter from the EU citizen saying that they will be travelling should suffice. Note that arrival Border Officers can and do check that the EU citizen is travelling or is already in the UK.

I do not see how dates of travel are at all relevant in any form of decision. I would personally leave it blank and explain in the notes that you left it blank because it is not relevant to any EU Free Movement decision.

Also consider contact Solvit. They take 8 weeks or so to do their thing, so starting early is a good idea.

If you were unmarried, then the clock starts ticking either when the Residence Card was issued OR when you entered the UK with your partner after the EEA Family Permit was issued.

I also tried to ask for the family permit this past July, but we couldn't go through with it.
What does this mean?

I do not think travel history has any relevance. You can put "Not Relevant."

Do you have a confirmation of your PR issued by the Home Office? If so then you have proof of your permanent residence. That is "unlimited right to remain in the UK". You can point out that you can apply to naturalize one year after getting either ILR or PR. Push back. Also get Solvit involved.

If my husband gets a RC via SS route, why would it be a problem boarding the plane with a A4 paper RC? As long as the A4 paper RC has not expired, I would expect that travelling with it should be the same experience as you have had previously travelling with it. Personally I would expect no problem...

Where you plan to live in the future is not relevant for this discussion.
Will you be travelling together with your husband to the UK?
How will you be travelling to the UK?
If you want to fly, then I suspect you will be refused entry to the plane unless you have some sort of UK visa.

Your rights under Sing do not expire. If travelling with your husband you should be admitted on arrival.

Unfortunately the whole process is a little slow. And many UK embassies ask for information that the British Home Office claims they have been instructed not to ask for.
