Separate PEO booking for main applicant & dependant on same

Hi,

I am on Tier 1 General applying for ILR this October.
I have booked PEO appointment for myself on 10/10/14.
I intended to do a postal application for my wife but as I have learnt that I have to submit my passport with her application I want her to apply with me on window. There are still slots available for 10th October but I am not sure whether I book her appointment separately but do one application on one SET(O) form for both.
Can someone enlighten me on this please?
