Set O or Set M form

Hi,

My ILR is due in 2 months.

I entered UK in Jan 2014, as dependent of Tier 1 General Visa holder.
My husband took ILR in JAn 2016, when I was granted PBS dependent leave to remanin Visa.

Now he is a British citizen and I have to apply for ILR 5 year route, should I use form Set O or Set M?
