Simple Form AN question

Guys,

I have simple question where to enter my first name in the following sections

1.5 should enter surname

1.6 i need to enter my first name

Is it correct or i am missing something


1.5 Surname/Family Name Please note: The name you give here will be the name shown on your certificate so
please ensure it is spelt correctly and you have written it in the correct order.

1.6 Other names
