some questions regqrding my extention under Tier1

I hold a HSMP visa and now applying for Tier 1 extension I am having problem on following areas in application form

Please help me to find the following information

1. Home office Reference no (Section 2: B-10)
2. Reference no. on Visa (section 2: D-4)
3. My Qualification is MSc Distributed Computing System
from Greenwich university, but when I try to look for this
cource on the point based system for Tier 1 I can find
Computer Science / IT in Masters Degree with my university
name so is it correct and my cource did come under it?
4. In Section 3c: T-5 my earnings are in UK so what should
i enter in exchange rate field?
5. In Section 3C: T-9 Should I enter accountant and full
company name and address for pay slip information? also
should I enter bank manager and bank address for bank
statement information?

1. Home office Reference no (Section 2: B-10)

This no. will be on your HSMP approval letter or on the letter which you received after HSMP stamping.

2. Reference no. on Visa (section 2: D-4)

Check your visa stamp page on passport. The no. on the top right corner. Something with "UKxyz" in red.

Should be fine assuming your university is affiliated.

Put N/A or 1

You should provide all information which HO can used to verify your documents.
