It means that if your relationship has permanently broken down that you need to either prepare to leave the UK or to find another category of visa to qualify for. At present, if your relationship is no longer subsisting, you cannot qualify for ILR after 5 years of a spouse visa (nor can you extend it).
A spouse visa is only really for remaining for the purposes of that relationship. Your (ex?) husband can/ should report the breakdown of your relationship to the Home Office, and they can then decide whether or not to shorten the length of your current visa (called curtailment).
