I hope you are well!
I'm completing the British citizenship application for my wife and have some questions
Some background - my wife came here on a spouse visa and then after about 2.5 years we applied for the visa extension after which she was granted ILR.
1/ What is the level of English required for the application? This is for the question "Does your degree meet the English language requirement for this visa?"
2/ I've been through and filled the form and went through to the documents required section and see there is a section for ecctis:
"Ecctis letter that confirms the qualification meets or exceeds the recognised standard of a Bachelor's or Masters degree or PhD in the UK AND confirm the level of English to which the degree was taught or researched"
I'm not sure this is required as my wife has passed IELTS and is currently working with the NHS.
3/ The degree certificate for --- to prove the level of English language required
Again as with the above - is this required?
4/ Have you made any previous UK immigration applications in the UK or abroad?
Should this be yes as we have made an application for FLR-M before (the visa extension)?

A degree taught in English is NOT required! The language requirement states you have to provide ONE of the following (from the pdf version fo the AN form). The fact that your wife works for the NHS doesn't matter.
I have a speaking and listening qualification in English at B1 CEFR or higher, that is on the Home Office’s list of recognised tests and was taken at an approved test centre (go to question 1.23).
Or
I have obtained an academic qualification (Bachelor’s or Master’s degree or PhD) in the United Kingdom.
Or
I have obtained an original degree certificate that was taught or researched in a majority English speaking country and:
• an Academic Qualification Level Statement (AQUALS) from Ecctis (formally UK NARIC) confirming the qualification is equivalent to a UK qualification
Or
an original degree certificate that was taught or researched in a non-majority English speaking country and both:
• an Academic Qualification Level Statement (AQUALS) from Ecctis (formally UK NARIC) confirming the qualification is equivalent to a UK qualification
• English Language Proficiency Statement (ELPS) from Ecctis (formally UK NARIC) showing that the degree was taught in English
Or
I met the knowledge of language requirement to qualify for settlement on or after 28th October 2013.
Or
I am a national of a majority English speaking country. (You must check the list of acceptable countries that are considered “a majority English speaking country”, to meet this requirement).
Or
I wish to claim exemption on the basis of my age.
Or
I wish to claim an exemption on the basis of a physical and/or mental condition (applying on this basis may not automatically lead to exemption. You should provide reasons
on page 23. Your application may fail and the fee be retained if sufficient reasons for
exemption are not provided).
If your wife has taken the IELTS test at an approved test centre and she achieved a B1 level, she needs to tick the first option and providing those details will suffice, more information here
