Switching from Tier2(General) to ILR

Hi
I will be completing my 5 years in Tier2 in April 2019 and eligible for ILR.
Could You Please let me know what all documents i need to do so ? I was on maternity leave from april 2018 and its finishing on March 2019 and I am applying for ILR by April 2019, so how do i show my payslips?
