Template for NHS letter and School letter for Child's ILR appln

Hi,
I understand, for my child (yr 7) 's ILR application, I need to include a reference letter from GP and a letter from School as proof of residence (or some other purpose).

Will it possible for someone to share a template for these two letters?.. I can get those letters accordingly as per the template..

Or if there is no template available, can you please let me know what are the important things that should be present in those two letters?.
