B1 test (Trinity college) certificate or online reference!

Hi everyone on this great immigration board.
I am planning to apply for my wife's application for ILR on a long residency basis.
I would like to ask about the online form,
1)she just passed her B1 test from trinity college, the online form gives me 2
options one of them is "CERTIFICATE" and other is "ONLINE REFERENCE NUMBER"
Which option I should tick? As I know her unique electronic reference number from the e-mail.
2)At the end of her online application, there is a mandatory list of documents
which ask to provide the last 2 years of evidence to live with your husband. As far as I know, set LR form is irrelevant to this requirement. I just confused about what should I do? should I provide the last two years evidence or just ignore it.
3)She lost her 1st passport when she came to this country in 2008, I know we have
to provide some evidence to cover this period such as bank statements,
hospitals letters and we also providing the police report for losing her passport.
What else we should provide to avoid any delay?
If anyone please can help me to fill the online form and type of documents we should provide, will be highly appreciated.
