Hi,

I am now living in the UK with my husband, and i have the family permit visa. Now I need to apply to a 5 years visa, is the correct form the eea21?

And after reading the form, I have noticed that they do not ask for my husband's bank statement, do I need to attach this document with the form?

Hi bjsr,

If your husband is an EEA / Swiss national and if he is residence in the UK in conformity with the EEA Regulations, you are entitled to receive a "Residence card of a family member of a Union citizen" upon application.

The correct form for this is EEA2.

Your husband's bank statement is not usually required, but it depends on his activities in the UK / whether or not he has acquired the right of permanent residence.
