Tier 1 application form query

Hello

I am applying (converting from WP) for Tier1 general from UK in Sep 2008 along with 3 dependents. I have 2 queries hope someone can help.

1. In dependent form - section 2 / D4 Immigration history, they are asking for the reference number for entry clearance or visa. Should this number be of the new visa which was issued in UK after renewal or my original one which was issued outside UK (Chennai, India).

The reason I am confused is because question D2 states - from within which country did dependent obtain entry clearance or visa before entering UK...

This same confusion exists in main applicant and dependent forms...

2. Also would appreciate some info from someone who has submitted single bank statement to show maintenance funds for main applicant and dependents - should i just attach bank statement with main application?? do i need covering letter to explain or how should i do it.

Also, can anyone tell me where i can find the reference number in the visa stamping in the passport...

It is the one that is mentioned on the right hand top corner of the visa. It has the format of "UKxxxxx".
