TIER 2 DEPENDANT FORM

Hi,
i have filled vaf10 application form,i have to ask some questions about that form.
1- wife's father date of birth is not confirmed.
2-other question ask about joining now or accompanying later(my wife is in Pakistan)
3-given name is just her name or my name as well.or my name just come under family name.(my name is her surname)
4-very important one i am submitting bank letter as maintenance fund evidence plus bank statement to show salary credited,my question is
Do i need to put cross on just in box of letter from bank or statements submitted box as well.please clear this.
5-in my covering letter i write: A bank letter confirming funds for three months i.e 17.03.2011 to 17.06.2011,they are 92 days. is this correct? or it must be from 19.03.2011 to 17.06.2011 which comes exact 90 days
