Tier 2 dependent form and premium service fee?

Hi,

I am going to apply tomorrow and I have 2 kids, I am on PSW at the moment and my wife is the main tier 2 general applicant. I want to ask about the submitting application, I cant find any option to apply online the only form I could find is the paper form this one https://www.gov.uk/government/uploads/s ... _07-14.pdf

Should I use that for myself and the kids?

Also my visa is expiring on 29th Oct, the homeoffice should receive my application on or before 29th right? I dont have to get a visa before that?

I also tried to check and apply through the premium service, but cant find how to apply for the family group and also the fee they are showing is £1600? shouldnt it be £1001? the only options I could see for the main applicant part is tier 2 general over 3 years and others for shortage occupation, but nothing under 3 yrs tier 2?

Please help with these things I've to apply within this week.
