tier 1 dependent visa

Hi,
I am currently in UK on HSMP visa.
I want to apply for dependent visa for my wife in India.
I came across the following application form.

Tier1 (Dependent)

Please can anyone confirm if it is the right application form. If not please can you provide me details regarding which application form to use for applying for dependent visa from India.

If you are currently on HSMP, then I believe that your wife may apply on form VAF2 (Guide).

Else, if you are currently on highly skilled worker (Tier 1 General), then see also What do my dependants need to make an application?
