Tier 2 (G) extension for self & dependents

Hi all,

I am going for my current Tier 2 G extension which is going to be expired in next 5 weeks. I need to apply for my family as well with 3 dependents. I'm planning to apply through standard route (not priority & premium) and have gone through many relevant posts. But its always confusing to get multiple views of some standard queries on this topic when as the corresponding threads goes on.

So , please excuse me and some answer my queries. Thanks a lot in advance


Should I submit 4 different application forms (page 1-43) for self plus 3 dependents
Do I need to submit anything else other than application forms (with COS number), photos & passports. (for example offer letter again, bank statement, utilitie bills etc, marriage certificate, birth certificates of kids etc
