Tier 1 extension

Hi
My tier 1 visa extn is due in Aug 2011.
Can some one suggest me where I can find application form for tier 1 extension. Which url link I can book in-person appointment?
When I called croydon fofice they said they don't know which application form is meant for tier 1 and they were not allowing me to book appointment in phone for tier-1 extn.
I am confused and tensed as I could see there are many changes are going in place.
Some one please guide me thru.
