My husband is going to apply for Tier 2 extension and our daughter and I will apply the extension at the same time as his dependants. This is the list I think we need to submit:
1. Passports
2. BRPs
3. Photos
4. Printed Application
5. My and my husband’s Bank Statements for the past 90 days
6. My husband’s payslips for the last three months
7. My husband’s employer letter
8. Degree Certificate (English language requirement)
9. Marriage certificate
10. Council Tax Bill in joint names
11. Water bill from company 1 in joint names
12. Water bill from company 2 in joint names
13. Electricity Bill in joint names
14. Gas Bill in joint names
15. Our daughter’s birth certificate
16. Doctor’s letter (our daughter)
17. School letter (our daughter)
18. Consent letter from us to our daughter
These are my questions:
(1) Did I miss anything?
(2) For my husband’s employer letter, what information needs to be shown in the letter?
(3) When I applied for my daughter’s visa in our country last year, the people in VFS centre asked me to submit my supporting document with these separators:

Do I also need these separators when I apply for the extension in the UK this time?
(4) Do I need to take photocopies of all the documents we are going to submit?
(5) For the utility bills, how many years I need to provide? Two years?
