tier 1 gen dependent extension in person appointment

Hi,

I got my ILR recently (I was on Tier 1 General), I am now planning to apply for my wife's Tier 1 Gen Dependent extension.. while filling the online form for in person appointment which option do we need to select for this..

Which application form are you using to make your application?*
1. PBS Dependent (Tiers 1,2 and 5) paper form
2. Tier 1 General
