Tier 2 General Visa Application form query

Hi,

Need help.

I am applying for tier-2 genera visa and while filling the form, I need some help:

There is section which asks about travel to UK in the past 10 years.

My situation:

My last entry into the UK was on tier-2 ICT visa. While in the UK I switched to PBS dependent of my wife.

My questions:

1) Does the above section asks how many times I entered in the UK?
2) Should my entry count as 1 which I entered as Tier 2 ICT migrant or should it count as 2 as I switched my visa from Tier 2 ICT to PBS Dependent.
3) if I don't specify PBS dependent then the last entry reason would remain as 'Work' and it would show incorrect details that I have stayed for so long for 'Work'.
