Tier 2 to health care worker visa

Hi. I have a BRP, on a tier general work visa. I work in NHS. I am continuing with same employer. They sent me COS. My current BRP expires in middle of August 2022.

I will be applying health and care worker visa now.
1) Which option do i choose? Is it extend or switch visa?

2) is Visa fee is just 247£ now?

3)How long is it taking for visa processing these days?

4) Are we still allowed to start work when visa extended is being processed?
