Tier-2 (ICT) extension on Maternity Leave

Hi. My wife is on Tier-2 (ICT) Long Term Visa and is going into Maternity Leave when her visa is about to expire.
Visa Expiry Date: May 20 2015
Maternity Leave Start Date: Feb 19 2015

My questions in this regard
1. When can she apply for an extension of her Visa considering that you can only apply 3 months before expiry
2. Does she need to provide details of her maternity leave when applying for an extension
3. As a fall back option, can she apply for dependent visa in-country? (her spouse-Me is also a WP visa holder)
4. Are there any other implications that need to be considered?
