Tier 1 Visa Extension

Hello,

My tier 1 expires in Jan 2013. I am currently in the UK working. To get my visa extended:

1. How long in advance should I apply?
2. Is it faster / easier to get it done in the UK (Croydon) or go back to my home country and get it done there (last time the processing was pretty fast in NYC)?
3. What paperwork do I need? Do I need to re-qualify for the points or if I am in the UK with a paid job etc should be all right.
