Tier 2 VISA Required Docs ?

Hi.
I am from Pakistan and completed my Bachelors degree from here. Currently I am working as a Software Engineer in a reputed company and have 1.5 yrs of experience in my field. Now, I have a job offer from the UK Software house that is ready to sponsor me.The company is also listed on fully approved (‘A-rated) category. I am willing to work in that company. Can somebody tell me what the the required docs and steps to apply for Tier 2 visa ?

I already have IELTS academic with 6.0 bands, is this okay ? or i need to appear in General Training IELTS exam?
