tier2 dependent form

my husband on tier1 visa n now switichign to tier2 with certificate of sponsorship. i have been looking for tier2 dependent form. one form i had completed has title t2(w) dependent version 10/11 but thing is that it doenst contain payment details section. another form i saw was pbs dependent which had payment details too. which is right form for tier2 dependent. plz reply me soon so that ican fill

If applying from within UK then the form given on this webpage.
If applying from outside UK then the form given on this webpage.
