1.) Your HR needs to go back to maths class - Nov 2012 until Nov 2013 is one year (12 months), so add another year - and they get to two years!
2) you can get your visa extended up to 3 years +14 days at each application. The length of an extension is determined by what the company put as a 'start date' and 'end date' on your CoS. Your company will need to issue a new CoS when you go to apply for your extension. The process for a visa extension is not very different from an initial application.... This means you will need to submit the application, pay the fee and go for a biometric appointment again.... etc...
3.) After 5 years in the UK (5 12 month periods) you will be able to apply for ILR, but you will need to meet the criteria for ILR. There is an earnings requirement for ILR applicants after 2016. You can be under Tier 2 for a max of 6 years to meet these criteria.
