Damaged UK passport

Hello senior member
I need some advise please. By mistake I washed my passport with cloths it has been damaged I booked the appointment for new passport next I was advised I need to fill up form please advise what exactly documents I need for replacement and do I need to take my damage passport with me on appointment

Passport Guidance Booklet - see page 12 and 13, point number 4 in the table.

P.S. this guide is included in the envelope from the Post Office with the form.
