payment form - AN and passport

Hi,

I am wondering whats the procedure of making a payment for:
i. AN form
ii. passport application

under the pilot scheme, (at the selected council running the scheme)

Do i need to fill in a form before hand?

one for each?

where can i download the payment form from?
