Regarding OCI

Dear all,
I have a two questions regarding OCI application. I already upload my picture online. Do i still need to take two set photographs? 

Do I need to submit two sets application forms and other documents?

U need to take 2 photographs as per the size guidelines.
Printed online application form
Declaration form
Appointment letter
One set of originals
One set of self attested photocopies
Hope it helps.
