Salaried and Self Employed Income FLR(M)

Dear Members, I am slightly confused with my wife's FLR M application and need your assistance.

I am self employed Income before Tax of £15000 p.a. for 2015/16

My wife is employed with income of £8000p.a. for 2015/16

The extension time is Dec 2016

I am confused that if I can combine my self employed income and her salary income to meet 18600/- requirement? If so, under which category.

Also, since the extension time is Dec 2016, do I need to show full year account for 2015/16 or 6 most recent accounts till Dec 2016 from salaried and non salaried employment?
