Form AN1 and passport - single name

Hi - my wife is applying for naturalisation and passport together via NCS. She only has one name i.e. Karishma without any surname in her current Indian passport.

What should we enter in the form AN1/passport form - Karishma in other name row and keep the surname/family name blank?
