stamp 4eufam to EU5

Hi All,
On behalf of my friend,
I (non-eu) got stamp 4eufam 5 years residence card in NOV 2016 on basis of my British national brother working in Ireland.
Brother (sponsor) is in Ireland last two years working. I as well working since nov 2016 after RC.

Now my brother wants to move to UK as he got married last year & our mom is in UK have serious health issues and needs our attention. In that case what options i have. I am eligible to apply form EU5.
If yes, what documents do i have to provide.

simple answer in NO , if EU national leave the state then you have no right of residence in the state as your right is dependent of his right . EU 5 is not for you as you haven,t lived in state even a year yet .
