Tier 5 visa reapplication

I'm currently under Tier 5 Temporary worker (Exchange) for almost one year and will expire in one week. Now I received another internship offer from another employer and need a new visa. Can I go back to my home country and reapply for the new Tier 5 visa with the new Certificate of Sponsorship?
