Application for a minor - form 9

Good morning,

I have 2 questions about an application for a minor.

1. What form should be used for the application of a minor (3yrs 5months) born in Ireland, if the father of child is a naturalized Irish citizen - form 9 (based on the father's Irish Citizenship) or form 11(based on birth in Ireland, but only after 5 years of child residency)?

2. If form 9 should be used, one of the documents from the checklist is:
"A letter from the Department of Employment Affairs and Social Protection stating that you are receipt of child benefit for your child, and the date on which payment commenced".

In this case, the mother (EU Citizen) is the receipt of child benefit and the father will complete and sign the Form 9. Can the fact that the father is not the receipt of child benefit be a problem for application?
The parents are married and living together with the child in Ireland.
