Question about Migrant change of circumstances form

Hey, I have just renewed my passport so need to update HO about this change of personal details.

I have got a question about the Migrant Change of Circumstances Form:

In the Form it does not ask what your current BPR number, or what you current passport number is. How does the Home Office know who this Form is for? (The Form does ask name and DOB but what if someone has the same name and DOB as you).

Sorry if this question seems thick but I am really wondering how they identify the right person via the Form.
