Hi,
My tier 2 dependent visa is due to expire in mid october. Before that my husband is applying for ILR.
I've been here for 3 years.
Could you help us with -
1. Do I get to extend under Tier 2 dependent or have to change my visa type to ILR dependent - if its later then do my 5 years start again?
2. Could you help with link to apply for Tier 2 dependent and also any reference (this forum or other sites) to documents required.

On what basis is your husband applying for ILR, 5 years Tier 2 or 10 years long residence?

5 year on Tier 2
Extend it on tier 2 dependent.

Or can I fill in the applicaiton and get an appointment date post his ILR appointment date?
You can but your application won't be processed until his ILR is granted. You also won't be able to pay the correct IHS for 3 years if his ILR is still pending.
When is he applying for ILR exactly??

Could you help with how much health surcharge would we have to pay - as on the
health surcharge calculation it asks for what is the visa expiration date for my husband - but given now he is on ILR, what date should we be mentioning in there?

You should put his ILR expiry date but not 2024. Put a date like 18 September 2022, ie. 3 years in order to get the correct IHS of £1200 per applicant.

Thanks for your reply - but still in doubt; as on the NHS surcharge site, it is asking for till what time is the Tier 2 visa for my husband valid for.
But my husband is no longer on it, so wouldn't putting any date in there giving wrong information?

So in the field where they are asking for visa expiry date - I just enter May 01, 2022 when I would be eligble for ILR.

You add the date 3 years from now or the date you apply. You get a 3 years extension and must pay 3 years IHS. You cannot choose a lesser time.
